New Patient Registration
Patients who live in our practice area may register with our practice.
When registering please bring:
- Your NHS number (your previous surgery will be able to provide this)
- Photo ID (e.g. passport, driving licence, student card)
- Proof of address (e.g. utility bill, bank statement dated within the last 3 months). Please note we do not accept mobile telephone bills.
- If registering a child please bring their red health book. To register a child at least one parent/guardian must be registered at the practice.
You will need to complete a registration form and we ask that you also complete a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. Both forms can be downloaded from the above "Registering Online" tab. Once we have received these, you will have to come to the surgery to sign to confirm that the details are correct and provide us with ID.
